Παρότι ο Κέβιν έχει εξωτερικεύσει 23 διαφορετικές προσωπικότητες στην έμπιστη ψυχίατρο του, τη Δρ Φλέτσερ, υπάρχει ακόμα μία, το «τέρας», που ελλοχεύει και είναι έτοιμη να πραγματώσει, αλλά και να κυριαρχήσει πάνω σε όλες τις άλλες. Εξαναγκασμένος να απαγάγει τρεις νεαρές κοπέλες, ο Κέβιν φτάνει σε σημείο να έχει μέσα του να αντιμάχονται αυτές οι προσωπικότητες, καθώς οι «τοίχοι» ανάμεσα τους αρχίζουν να διαλύονται.
