Ο Max Payne είναι ένας μυστικός αστυνομικός, ένας μυθικός αντι-ήρωας, που προσπαθεί να εντοπίσει τους υπεύθυνους για τους κτηνώδεις φόνους της οικογένειάς του και του συνεργάτη του. Τυφλωμένος από τη μανία του για εκδίκηση, η έρευνα τον οδηγεί σε ένα εφιαλτικό ταξίδι στον σκοτεινό υπόκοσμο. Ενώ το μυστήριο περιπλέκεται, ο Max αναγκάζεται να πολεμήσει εχθρούς πέρα από τον φυσικό κόσμο και να αντιμετωπίσει μια αδιανόητη προδοσία..
