Κατά τη διάρκεια ενός ανέμελου week-end με τους φίλους της, η νεαρή Κίμπερλυ οραματίζεται ένα φρικτό αυτοκινητικό δυστύχημα, στον αυτοκινητόδρομο 23, το οποίο όντως συμβαίνει λίγο αργότερα, αλλά χάρη στην κοπέλα, αρκετοί άνθρωποι σώζονται από βέβαιο θάνατο.Και αυτοί όμως, οι ελάχιστοι διασωθέντες απειλούνται από τον. Θάνατο, ο οποίος επιστρέφει δριμύτερος, για να διεκδικήσει με κάθε τρόπο, τις ζωές που του ανήκουν!Τώρα, καλείται η Κίμπερλυ, να ενώσει τις δυνάμεις της μαζί με την Κλαιρ (η μόνη διασωθείσα από την προηγούμενη ιστορία) που ζει απομονωμένη σε μια κλινική, με μόνιμα φωλιασμένο το φόβο του θανάτου στην ψυχολογία της, ώστε να προστατεύσουν όλους όσους κινδυνεύουν θανάσιμα.Το τελικό συμπέρασμα του έργου είναι πως τελικά λίγοι ή μάλλον ελάχιστοι μπορούν να ξεγελάσουν το θάνατο.
