Θέμα της ταινίας είναι οι εφιαλτικές 127 ώρες που έζησε ο ορειβάτης Άρον Ράλστον, όταν ένας βράχος έπεσε πάνω στο χέρι του, παγιδεύοντάς τον σε ένα φαράγγι στη μέση του πουθενά. Στη διάρκεια της περιπέτειάς του, ο Ράλστον θυμάται τους φίλους του, την αγαπημένη του, την οικογένειά του και τις δύο πεζοπόρους που γνώρισε πριν το ατύχημά του.
