Δύο πολύ όμορφες νεαρές αμερικανίδες ταξιδεύουν στην Ευρώπη. Ένα βράδι, ξεμένουν απόλάστιχο του αυτοκινήτου σε ένα δάσος στη Γερμανία. Ενώ ψάχνουν για βοήθεια βρίσκουν μια απομονωμένη βίλα. Την επόμενη μέρα τις βρίσκει παγιδευμένες στο τρομακτικό υπόγειο ενός εφιαλτικού νοσοκομείου μαζί με έναν Γιαπωνέζο. Ένα ηλικιωμένος Γερμανός βλέπει στον εαυτό του έναν συνταξιούχο χειρούργο ειδικευμένο στο διαχωρισμό Σιαμαίων. Ωστόσο οι τρεις "ασθενείς" του δεν πρόκειται να διαχωριστούν, αλλά να ενωθούν σε μια φρικιαστική λειτουργία. Σχεδιάζει να είναι ο πρώτος που θα συνδέσει ανθρώπους μέσω του γαστρικού συστήματος ώστε να πραγματοποιήσει την αρρωστημένη φαντασίωση μιας ζωής: την "ανθρώπινη σαρανταποδαρούσα"...
