Καλοκαίρι 1954. Ο Αμερικανός αστυνόμος Τέντι Ντάνιελς κι ο νέος συνεργάτης του, ο Τσακ φτάνουν σ’ ένα νησί κάπου στα ανοιχτά της Αμερικής, όπου βρίσκεται το Άσκλιφ, το ψυχιατρικό νοσοκομείο για ψυχασθενείς εγκληματίες, για να διεξάγουν έρευνα για μια ασθενή που φαίνεται να δραπέτευσε μυστηριωδώς. Όταν ξαφνικά ξεσπάει μια καταιγίδα, ο Ντάνιελς βρίσκεται αποκλεισμένος εκεί και η διακριτική γραμμή μεταξύ πραγματικότητας και παράνοιας αρχίζει να γίνεται επικίνδυνα δυσδιάκριτη. Στο νοσοκομείο Άσκλιφ, τίποτα δεν είναι τελικά όπως φαίνεται και ο Ντάνιελς βρίσκεται έρμαιο σε «απάνθρωπες», ριζοσπαστικές θεραπείες που περιλαμβάνουν μεταξύ άλλων πειράματα με φάρμακα, βάρβαρες χειρουργικές επεμβάσεις και πλύσεις εγκεφάλου… Όσο περισσότερο πλησιάζει στην αλήθεια, τόσο πιο θολή γίνεται αυτή, και τόσο περισσότερο αρχίζει να πιστεύει ότι μπορεί να μη φύγει ποτέ από εκεί… Τουλάχιστον όχι σώος, γιατί κάποιος εκεί μοιάζει να προσπαθεί να τον τρελάνει…
